Root Cause Analysis (RCA) is an internationally recognised tool for the detailed investigation of incidents. Its use is widespread but a variety of complex and conflicting methods have resulted

in the incorrect application of this essential learning tool. Training staff in the correct application of RCA techniques will assist in solving problems at the root rather than just fixing the

obvious. DNV GL provide risk management training for providers of healthcare, offering extra guidance to assist in engaging relevant staff and spreading ownership of risk management through all levels of the organisation.


What does Root Cause Analysis training include? 

Our unique approach to RCA training has a flexible structure to make it relevant to current practices and procedures within your own organisation. The course can include up to three distinct modules covering: 

• Benchmarking the organisation’s safety culture - presentation and guided navigation of a benchmarking tool to enable staff to apply it in practice and develop an action plan to address outcomes.

•Problem solving – a practical exercise to review work on one or more of the organisation’s incidents.

•RCA – a practical session that will examine in detail the requirements for applying and conducting a robust RCA to the investigation of incidents, complaints and claims. It will introduce and include the direct application of relevant tools.
